How to Utilize
Your Benefit
This laser vision benefit is available to all members and their eligible dependents. Follow these simple steps to obtain your discount:
- Please call toll-free 1-877-644-9090 to find a participating laser vision correction provider or Schedule Online at a LasikPlus Vision Center near you.
- Obtain a Discount Certificate (Not required if you schedule with a LasikPlus Center).
You may choose to receive a certificate via email. Simply provide our representative with an email address and you will automatically receive your certificate via email within 24 hours.
- Schedule an appointment with an "in-network" provider.
Please bring your Discount Certificate to the surgeon's office.
- Schedule a treatment date.
If you and your provider determine that laser vision correction is right for you, schedule an appointment with the provider.
- Payment.
The in-network provider will simply deduct your discount from the price. Financing arrangements, if available, may be discussed directly with the surgeon's office. There are no claim forms to file.
- Post-Operative Visits.
Please follow all post-op instructions and attend all suggested follow-up visits.
